FORT MYERS, Florida – The step by step to follow was the process we experienced on January 20th and 22nd.

This information will help you when you sign in to the portal. Waiting in line can be frustrating, but once you start, you should quickly go through the application process.
REMEMBER: You have not guaranteed a place until you reach the end of registration and confirm.
On the first page, you will need your
– Name
– Cell phone number
– Email address (this is important because they will send you an email confirmation)
– Date of birth
– Address
– Medicare number, if you have one
– You will be asked “Are you a Publix member?”
Once these instructions are completed, you will be asked to choose the location of the store where you would like to be vaccinated.
REMINDER: You’re racing against the clock, so know where you want to go and don’t be too picky. Just confirm that you are in the correct county you selected.
The store will open and you will have several options for your opening hours. Choose quickly and don’t waste your time.
You will have to answer 14 questions about your health. Here they are literally. Read now so you can move forward quickly:
- Emergency contact name
- Emergency contact number
- Are you 18 or older?
- Do you have a cold, fever or acute illness?
- Do you have allergies to medications, food or a vaccine? If so, please list.
- Are you allergic to chicken eggs or egg products?
- Are you allergic to Timerosal (cleaning products or contact lens solution)?
- Have you ever had a serious reaction after receiving a vaccine?
- Have you ever been diagnosed with Guillain-Barre syndrome?
- Do you have a seizure, brain or nerve problem?
- Are you the patient? If not, enter your name and relationship to the patient.
- Are you attending a clinic? If yes, please enter the location?
- Would you like to be notified about other immunizations?
- Have you ever received an initial dose of the COVID-19 vaccine?
If the selected time is up, you will be asked to choose a new time. If the store you selected has no further appointments, you may be asked to choose a different store.
After ensuring your schedule, you haven’t finished. You will be given a page to review your information. You should also click on two boxes at the bottom of the page. The first is a consent box that you are the patient or the patient’s representative. The second is a box that says “I am not a robot”.
Click Continue and you will see a screen that says “Thank you, (your name), your appointments are reserved” and an appointment identification number. It will include the dates and times of your first and second appointment. You will also have the address of the store you selected to do the vaccination.
Print the final screen for security – and then check your email for the confirmation message.
THERE IS ANOTHER STEP IF YOU NEED TO MARK ANOTHER PERSON !!!!
At the bottom of the confirmation page, there is a link that says “Make another appointment”.
You are in the system now and can schedule someone else’s appointment without joining the queue again.
