Did you lose your COVID vaccine card? Here’s what you do.

UTICA, NY — Every person who is vaccinated receives a vaccination card from the CDC with information on which vaccine you made and when you received it. “At least in the coming summer months, it will be a very important card to have alongside other documentation. Especially if different locations or other areas of work require it, ”said Picente.


The state now requires large venues to require people to provide proof of a negative test or vaccination in order to enter. Your COVID vaccination card or the New York State Excelsior Pass are two options that you can present.

If you lost or lost your card, don’t worry. The state keeps a record of your vaccination information in a secure database called the New York State Immunization Information System. This means that your cards can be replaced by calling the New York State vaccination hotline (1-833-697-4829).

For those who know about technology, the New York State Excelsior Pass may be an option for you. It is a new electronic application on your phone that allows you to present your vaccination data or negative vaccination results. The application is available on Apple and Android devices and is completely free. The application is not required now, but it may be seen locally in the future.

Experts warn against laminating their cards to protect them. They said the lamination could damage the ink that shows the vaccine information. This would prevent people from reading it. Lamination would also avoid future edits of the card, if more photos are needed in the future. The CDC card has two additional lines below it labeled “other”.
