Vaccination Registration Card

When patients receive their first dose at the Department of Health, they receive a Vaccination Registration Card with the following important information.

· The vaccine manufacturer (Moderna or Pfizer)

· The date the first dose of the vaccine was given

· The date the second dose of the vaccine is due

It is important to keep the Vaccination Registration Card, because when it comes to scheduling the second dose of the vaccine, it helps to ensure that you receive the correct product and on time. The current recommendation is that second dose vaccinations are received on or after the recommended date on the Vaccination Record Card, not earlier.
