A 91-year-old Ohio amputee went into shock, passed out, and nearly died after accidentally receiving the second dose of the COVID-19 Modern vaccine twice, just four hours apart.

Victor Smith, from the Cincinnati suburb of Hamilton, received the first dose of the Modern vaccine at Fort Hamilton Hospital in January, according to the Cincinnati Enquirer.

He fetched the second dose of the vaccine at the Butler County Fairgrounds on February 25, wrote his daughter, Dawn Smith-Theodore on Facebook.

On the fair grounds, Smith received a vaccination card with his first name written there, but an incorrect surname, according to his daughter.

“It turns out it was a Victor, but it was a different surname,” said Smith-Theodore.

After receiving the second dose, he was sent to the Jamestowne Rehab facility in Hamilton, managed by Community First Solutions.

Four hours after receiving the second shot, a fire department officer entered the facility to administer the vaccine to a man of the same name.

‘At 3:45, a fireman from the fire department came in and said he had a chance for Victor, and they said,’ Victor Smith, ‘and he said’ yes ‘,’ Smith-Theodore told WKRC-TV.

“The patient was transported to the hospital and the family was immediately contacted.

‘Both Jamestowne and the Hamilton City Fire Department began investigations into the incident.

‘Our thoughts are with the patient and his family as he continues his recovery.’
