500 doses of vaccine discarded in clinic due to ‘human error’

MILWAUKEE (AP) – Doctors had to discard about 500 doses of the Modern vaccine after the vials had been kept without refrigeration for a long time at Aurora Medical Center in Grafton.

Advocate Aurora Health officials said someone removed 50 bottles from a refrigerator to access other items and did not put them back overnight on Friday.

Each vial contained 10 doses of vaccine. An internal investigation concluded that the failure was an “unintended human error”.

The doctors even managed to administer part of the vaccine from the vials within the allowed 12-hour post-refrigeration window. Once the vaccine is thawed, it cannot be refrozen.
